Attention Washington and Lee Athletes:

In accordance with NCAA Sports Medicine Guidelines, Washington and Lee University is requiring that all first-year University athletics participants have a physical examination on file with the University Health Center. All returning athletes will complete a detailed health history form and have to undergo an updated physical examination if deemed neccessary based on their detailed health history form.

For returning Washington and Lee University athletes, a physical exam is not a general requirement unless for the above stated reason. Please return the completed documents listed and linked below to the following address:
